Registration is now open for this important industry event!

To register for the 2017 Leadership Forum, complete our Online Registration Form below.

You can register up to 4 delegates per transaction.

Fees:

Registration for up to two – $695 (USD)

After the first two registrants  – $495 (USD)

Please Note: If you are registering more than four people and would prefer to be invoiced, please email us at events@naphia.org.

Form Problems: If you experience any difficulties with online registration, please contact us at events@naphia.org and we will arrange to invoice you.

Registration Deadline: May 8, 2017


When form is complete, only press the “Register Now” button ONCE. The transaction may take 10-30 seconds to submit. Pressing the button twice may cause a duplicate transaction.

Cancellations:

Cancellations received in writing prior to May 8th, 2017 will be refunded the full amount less a $50.00 administrative fee. Cancellations received after May 8th, 2017 are non-refundable. If you are unable to attend, we encourage you to find an alternate registrant to fill your spot. Changes to your registration name must be received in writing by May 8th, 2017. No refunds will be provided to registrants who do not attend the Leadership Forum without prior written notification of cancellation by May 8th, 2017.
